What is All-on-4?
All-on-4 is a full arch implant technique that replaces an entire row of missing or failing teeth using just four strategically placed implants. Two implants are placed vertically at the front of the jaw, and two are angled at up to 45 degrees at the back. This angulation maximises contact with available bone, often eliminating the need for bone grafting entirely.
The key advantage is immediate loading: a fixed temporary prosthesis (a complete set of teeth) is attached to the implants on the same day as surgery. You leave the clinic with functional teeth. The permanent prosthesis, typically fabricated in zirconia for its strength and natural appearance, is fitted after the implants have fully integrated with the bone (3 to 6 months).
All-on-6 follows the same principle but uses six implants per arch for additional load distribution. This is often recommended for the upper jaw where bone density is typically lower. For patients with severe bone loss in the upper jaw, zygomatic implants (anchored into the cheekbone rather than the jawbone) are available as an alternative to bone grafting. Your surgeon recommends the right approach based on your 3D CBCT scan.

How long does the All-on-4 surgery take?
The surgical phase typically takes 4 to 6 hours per arch under local anaesthesia with sedation. This includes extraction of remaining teeth (if needed), implant placement, and fitting of the immediate temporary prosthesis. You leave the clinic the same day with a functioning set of teeth.
When do I get my permanent teeth?
You receive a fixed temporary prosthesis on surgery day or within 48 hours. This is fully functional for eating and speaking. Your permanent prosthesis (typically zirconia or high-grade acrylic) is fabricated after osseointegration completes, usually 3 to 6 months later. This can be fitted during a short return visit to India or by a local dentist using the records we provide.
Can I eat normally after All-on-4?
For the first 2 to 3 weeks after surgery, you will be on a soft diet while the surgical sites heal. After that, the temporary prosthesis allows normal eating with some caution around very hard foods. Once your permanent prosthesis is fitted, you can eat without restriction. Patients routinely report being able to eat foods they had avoided for years.
What if I need bone grafting before All-on-4?
One of the advantages of the All-on-4 technique is that it often avoids bone grafting entirely by angling the posterior implants to maximise contact with available bone. For patients with severe bone loss, zygomatic implants (anchored in the cheekbone) are an alternative that also avoids grafting.
